We are looking for a competent Clinical Pharmacist who will ensure the rational and effective use of medication. You will perform various functions through collaborative drug therapy management (CDTM) and offer excellent patient care.

A clinical pharmacist's role can vary depending on the setting they work in. However, generally, here are some of the tasks a clinical pharmacist may perform on a day-to-day basis:
Medication Reviews: Conducting reviews of patients' medications to assess appropriateness, effectiveness, and possible side effects. This may involve reviewing medical charts, speaking with healthcare providers or patients, and making recommendations for therapy adjustments.
Medication Management: Collaborating with healthcare teams to develop medication management plans, including selecting the most appropriate medications, determining dosages, and monitoring therapeutic outcomes.
Patient Consultations: Providing patient education and counseling on medication use, potential side effects, and treatment plans. This may involve answering questions, addressing concerns, and ensuring a patient's understanding of their medication regimen.
Drug Information: Researching and providing evidence-based drug information to healthcare professionals, such as physicians and nurses, regarding drug interactions, adverse effects, and new therapies.
Clinical Rounds: Participating in rounds with healthcare teams, discussing medication-related issues, and making recommendations for optimizing drug therapy based on patient-specific factors and medical evidence.
Quality Assurance: Monitoring medication safety practices, participating in medication error reporting and prevention, and implementing quality improvement initiatives to ensure safe and effective medication use.
